These forms are NOT for individuals interested in applying for any Pinellas County Housing Authority program. You cannot apply for the waiting list by completing any of these forms. These forms are for existing housing choice voucher participants and housing choice voucher landlords.

Authorization of Release HUD form 9886 | Tenants and Homeownership Participants Tenants complete this form once a year to give the Pinellas County Housing Authority permission to obtain income information from current and former employers, unemployment, IRS and financial institutions (i.e. banks, credit unions, etc.). All household members 18 years or older are required to sign this release form at least once per year. |
Authorization of Release of Information | Tenants and Homeownership Participants Tenants complete this form once a year to give the Pinellas County Housing Authority permission to obtain criminal record, identity and marital status, employment income, pensions, assets, federal state or local benefits, child support, student status, disability assistance expenses, medical expenses, utilities and housing history information from any individual, organization, or agency, including any governmental agency. All household members 18 years or older are required to sign this release form at least once a year. |
Certificate of Repairs | Landlords Landlords complete this form when they have had an inspection that required repairs and the repairs are now complete. Once the landlord submits this form to the Pinellas County Housing Authority, a re-inspection will be scheduled to verify the repairs. |
Change Form | Tenants and Homeownership Participants The formal name of this form is the Interim Change Form. Current tenants complete this form to report any and all changes in household income, family composition (adding or deleting household members) or child care expenses. Applicants would also complete this form to report changes in their mailing address. |
Citizen Status 214 | Tenants and Homeownership Participants Tenants complete this form for each member of the household to indicate how that member is lawfully within the United States. This form is completed whenever a new member of the household is added. |
Debts Owed Form | Tenants and Homeownership Participants Adverse information is collected on all participants once housing participation has ended, whether the household voluntarily or involuntarily move out of an assisted unit. PHA’s are required to report this adverse information to HUD. This notice provides you with information on what information the PHA is required to provide HUD, who will have access to this information, how this information is used and your rights. PHAs are required to provide this notice to all applicants and program participants and you are required to acknowledge receipt of this notice by signing page 2. Every household member 18 years and older must sign this form. |
Disability Verification Form | A household with a disabled member should complete this form to request an accommodation that is not the standard policy or procedure for the housing authority. |
Family Self-Sufficiency Interest Form | Tenants and Housing Choice Voucher participants complete this form if they are interested in joining the Family Self-Sufficiency program. |
Fraud Complaint Form | Anyone Any person with information regarding a tenant or a landlord that they believe is not following the Housing Choice Voucher/Section 8 rules can complete this form. They may give their contact information or remain anonymous. Click here and print the Fraud Complaint Form on the next page. |
Fraud Notice | Tenants and Homeownership Participants Participants complete this form and acknowledge the penalties resulting from fraud. |
Homeownership Annual Checklist | Homeownership Participants Participants can use this form to make sure they have completed all the necessary documents required for an annual packet. |
Homeownership Responsibilities | Homeownership Participants Participants use this form as a guideline of best practices which helps the participant stay in good standing with the Pinellas County Housing Authority. |
Landlord Packet | Landlords This packet provides landlords with an information that explains the program in-depth and gives guidelines for participation. |
Landlord Contact Information Form | Landlords Landlords must complete this form to keep their contact information accurate. |
Lead-Based Paint Notice | Tenants and Homeownership Participants Participants acknowledge that their house, if built before 1978, contains no known lead based infractions. |
Live-in-Aide-Application | Tenants and Homeownership Participants Tenants who require a live-in-aide need the live-in-aide to complete this application. |
Medical Equipment-Disability Verification Form | Tenants Under certain circumstances, a tenant may request an additional bedroom for medical equipment as a reasonable accommodation if it the equipment is determined to be a medical necessity and it is large enough that it cannot be stored in other areas of the house. |
Monthly Expense Form | Tenants Tenants that claim little to zero income must complete this form so the Pinellas County Housing Authority can analyze expenses and income. |
Mutual Termination Agreement | Tenants and Landlords Tenants and landlords who mutually agree to terminate their existing lease and HAP contract must complete this form. It is your responsibility to complete the form and obtain the other party's signature. |
No Income Statement | Tenants and Homeownership Participants Tenants must complete this form if they are currently not receiving income from any source. Members of the household who are 18 years or older must also complete this form if they are currently not receiving income from any source. |
Notice of non-renewal | Tenants Tenants must complete this form if they choose not to renew their existing lease with their landlord. Landlords must sign and acknowledge this notice. It is your responsibility to complete the form and obtain the other party's signature. |
Payment Standards | Tenants and Homeownership Participants. This notice details the new calculation methodology of PCHA HCV Department and how certain payment standards may be decreased. This notice requires participant signature anytime the payment standards are changed. |
Personal Declaration of Eligibility | Tenants and Homeownership Participants Tenants complete this form in order to determine eligibility whether it be initial eligibility in order to receive a voucher or continuing eligibility to maintain an existing voucher. Tenants must provide household composition, income, assets, deductions, and criminal history about themselves and all members of the household. |
Portability Request Form | Tenants Tenants complete this form when they want to move to a location that is outside the coverage area of the Pinellas County Housing Authority. |
Reasonable Accommodation Notice | Tenants If you can show that you have a disability that interferes with your use of housing, if your request is reasonable, and your request can provide a nexus between the request and disability, we will try to make the changes you request. |
Reasonable Accommodation Request Form | Tenants and Homeownership Participants A household that has a disabled member would complete this form to request an additional item that is not the standard policy or procedure of the Pinellas County Housing Authority. |
Rent from Relative Form | Tenants This program does not permit tenants to rent from relatives except in special circumstances involving a reasonable accommodation for a disability. Tenants must show that no other units are available and/or can meet their needs. The proposed unit must have unique modification features customized to the tenants needs. In addition, the owner must not be living at the property. |
Rental Increase Request Form | Landlords A current landlord who wants to increase the rent on their unit would complete this form. This form must be submitted 60 days prior to the lease renewal date and must be signed by the current tenant. It is your responsibility to complete the form and obtain the other party's signature. |
Self Employment Declaration | Tenants and Homeownership Participants Tenants complete this form when there is no other way for you to report your income. |
Statement of Homeownership Obligations | Homeownership Participants Participants must follow HUD's obiligations as a homeowner and program participant. |
Tenant Annual Appointment Checklist | Tenants Tenants can use this helpful checklist to make sure they provide all the necessary documents to complete an annual packet. |
Tenant Annual Packet | Tenants This packet is a collection of forms including a checklist to aid the head of household with producing additional items at their annual appointment, a fraud notice, the HUD 9886, the lead based paint form, the tenant responsibilities form, the PCHA information authorization form and the portability form. Each household must complete this packet annually for recertification or for new moves. The head of household must complete and sign each form in this packet. |
